Immunization and exam requirements for PHS students:
Tetanus Vaccine- 3 doses
Polio Vaccine- 3 doses
MMR- 2 doses
Hepatitis B vaccine- 3 doses
Mantoux Tuberculin skin test- required for all students coming from countries with high incidence of tuberculosis as defined by the NJ Department of Health and Senior Services.
Physical Examination is required for all new students.
Please note additional changes:
- Tdap vaccine (Tetanus, diphtheria, acellular pertussis)
Beginning September 1, 2008 every child born on or after January 1, 1997 and entering or attending Grade 6 shall have received one dose of Tdap given no earlier than the 10th birthday and after five years have elapsed from the last DTP/DTaP or Td dose.
As of September 1, 2008 children born on or after January 1, 1997 and transferring into a New Jersey school from another state or country shall have received one dose of Tdap, provided at least five years have elapsed from the last documented Td dose.
Beginning September 1, 2008 every child born on or after January 1, 1997 and entering or attending Grade 6 shall have received one dose of a meningococcal-containing vaccine, such as the medically preferred meningococcal conjugate vaccine. (Applies to students when they turn 11 years of age and attend 6th grade)
As of September 1, 2008 every child born on or after January 1, 1997 and transferring into a New Jersey school from another state or country shall have received one dose of meningococcal vaccine.
