All students entering grades 9 and 11 should have a physical within a year of the first day of school. Complete Parts A & B
All students participating in a sport must have a current physical exam (valid for 365 days). |

Physical Forms |
|
All students participating in a sport must have a current physical exam (within 365 days). This complete form must be returned completed and and dated within 60 days of the start of the sports season. |
|
All students participating in a sport must complete and date this form within 60 days of starting EACH sports season. |
|
All students participating in subsequent sport seasons, must complete and date this form within 60 days of starting a new sport. (Must have previously submitted a current physical (Part A and B) |
